Circle of Hope provides services for 24 hours a day, seven days a week. 

These services may include:

  • Skilled nursing care
  • Medications related to hospice diagnosis
  • Medical equipment and medical supplies
  • Personal care services (including bathing, dressing, and feeding)
  • Spiritual care services and support
  • Social services and support
  • Bereavement care
  • Dietary Counseling
  • Physical, speech and occupational therapy
  • Volunteer support and services

 

     
 

Admission Criteria:

To be eligible for Circle of Hope services, patients must agree that they wish to receive palliative rather than curative care.  The attending physician and the Circle of Hope team physician must also agree that the patient gas an estimated life expectancy of six months or less, if the illness run it’s normal course.  The Circle of Hope staff conducts patient evaluations to help determine if hospice services are appropriate for all referred patients.

     
   

Circle of Hope recognizes that psychological, spiritual, and social services, as well as medical / clinical care, can contribute to an improved quality of life for patients and their families.  Every patient is served by a team of dedicate Circle of Hope professionals who are guided by the choices and decisions made by the patient, family and physician.  The Circle of Hope team consists of, but is not limited to:

  • The patients physician
  • A patient care manager
  • Registered nurses
  • Certified nursing aides
  • A medical social worker
  • A chaplain
  • Trained volunteers
